Printable Witnessing Survey

This survey was distributed at the 1997 Great Lakes S.A.L.T. Conference for the purpose of witnessing at a shopping mall in Indianapolis. I have reproduced this form in a format that should be easy to print from your web browser. Just click on the link below to view the survey and print it from your web browser. Click on your browser's "back" button to return to our web page.

It may be necessary to adjust some properties in order to print this form correctly (HINT: Look at the Print Preview before you try to print it). You will most likely want to turn off the optional printing items such as Document Title, Document Location, Page Number, Page Total and Date Printed. These can be accessed in the Page Setup (under the File menu) if you are using Netscape. Set smaller margins if the entire page does not fit on your paper. Set the font (in your web browser preference settings) to "Courier New" or "Courier" if the rows don't appear to line up nicely.
